Dear Patient,

Welcome to our Practice! We are grateful and honored to have you as our
patient, and look forward to a long relationship.

In order to improve quality of care for our patients we have prepared an
overview of policies used in our practice. Please look over it in your spare
time. We welcome any questions or suggestions that you may have.

Policies and Goals:

Deliver the best and safest care to our patients

We strive to provide you with the timely service, exploring medical        
problems in as much detail as necessary and provide you with complete
recommendations and guidance. We also believe strongly in patient
autonomy, which means that you are THE BOSS, and ultimately it is your
responsibility to decide if you agree with our recommendations regarding the
necessity of procedures or treatment options, after considering all risks and
benefits. Please feel free to express any concerns or reservations about
suggested treatment or course or action that you may have. Also, if you feel
that you need a second opinion about a particular problem, we would be
happy to refer you to a major clinical center such as Mayo Clinic.

If you as the patient and your physician agree on the course of treatment,
you should follow it, and not discontinue medications or treatments without
notifying your physician first. Stopping medication may be dangerous, as
some of them require slow taper, or observation. Also, please do not change
the dose of any medications that you are taking, without discussing it first with
your physician. This is essential in order to protect you from harm

Appointments

We will make every attempt to see you on your scheduled appointment time,
however in medicine nothing is for certain, and a patient may require more
time then was initially allotted (patient care comes first). Every effort will be
made for physician to see as soon as possible.

If a patient is late for his/her appointment, it will interfere with the rest of
scheduled appointment for that day, and cause delays in care for other
patients who may need it. Therefore, we ask all of our patients to arrive at
least 10 minutes prior to their appointment time in order to fill our paperwork.
If patient is late more then 10 minutes to their appointment, they may have to
wait for an opening in physician schedule (we cannot guarantee, that such
opening would become available) in order to be seen. Tardiness to more
then 4 appointments per calendar year may be grounds for dismissal from
the practice.

If patients cannot keep their scheduled appointments, please notify our office
at least 24 hours in advance, so another patient may be scheduled instead.
Missed appointments without notification take away time from other ill
patients who need timely evaluation and treatment. There is a charge of $10
for appointments missed (without 24 hour notification). Three missed
appointments (without 24 hours notification) may be grounds for dismissal
from the practice.

We will make every effort to see acutely ill patients who do not have
appointments on the same day, however we cannot guarantee it, if schedule
for that day is full. If it is a life threatening emergency, we would recommend
for patients to proceed to emergency room
Referrals and Consults

We would do our very best to refer patients to qualified specialists if needed.
It is a responsibility of patients to arrange for an appointment with the
specialist and arrange for records of any procedures or treatments to be
available to our office for review.

Hospital Admissions

Modern medicine is becoming increasingly complex, and we feel that it would
be unsafe for patients to be admitted to the hospital and be cared for by
physician who is also seeing patients in the clinic. Patients in the hospital
need careful and constant attention, and their problems have to addressed
in a timely fashion. Therefore we made an arrangement with the group of
hospitalist physicians to admit our patients to the hospital. (Hospitalists are
internal medicine physicians whose specialty is hospital medicine. They do
not see patients in the clinic and are available to care for patients in the
hospital 24 h/day.) After the discharge, we will receive all the records from
the hospitalization, and will continue patient care.

Office Hours

Our office hours are 8:00 am to 4:00 pm Monday through Friday. If you
develop a life threatening emergency, you should call 911 and proceed to
the emergency room. (Examples of life threatening emergency include, but
are not limited to chest pain, difficulty breathing, high fever, sever abdominal
or pelvic pain, speech change, loss of sensation, loss of vision, severe
headache, blood in stool, vomiting blood etc.) If you are unsure of what to
do, please call our office.

Prescriptions

Prescriptions will be refilled by our office in a timely manner. Please request
your pharmacy to fax us a prescription refill 24-48 hours in advance.

Please note that as our office grows, patient may not be able to speak with
our medical assistant immediately. Please feel free to leave a message with
our front desk, and we will return your call as soon as possible.
